As a Community Care Coordinator, you will play a pivotal role in our CPCT, focusing on delivering top-notch coordination to enhance the health outcomes of complex patients. You’ll be working with a team that values person-centred and whānau-centred care, making a real difference in the lives of those requiring seamless care across multiple providers. This is a 12 month fix term role (through to 30 June 2026)

• Conduct patient and whānau interviews to assess and establish care plans.
• Determine patient and whānau wellbeing goals.
• Identify and navigate multiple providers involved in individual care.
• Proactively monitor health, supporting self-management and preventing re-hospitalisations.
• Facilitate interdisciplinary functions at a General Practices, collaborating with health and social service providers.
• Provide support, education, and case management with an emphasis on intensive support when necessary.

We invite applications from Registered Nurses and Social Workers with primary care experience. The following qualities will be preferred:

• Registration as a health professional with a current practicing certificate.
• Strong cultural competency and a comprehensive understanding of Te Tiriti o Waitangi for guiding and its practical application in guiding initiatives within primary care.
• Ability to thrive both independently and in a collaborative team environment.
• A positive “can do” attitude with a commitment to achieving the best health outcomes.
• A continuous learner, always eager to enhance clinical skills.
